Re-Enrollment Form

If you have previously enrolled your child in a lesson with us, we already have your information in our database, so please scroll down to use this simpler form to let us know that you want your child to be in the same program for the next session. If you have new information, such as an additional child or lesson, a new address or phone number, etc., please put that information into the message part of this form. That way your child is re-enrolled and our database is up-to-date!

If you have not previously enrolled your child in a lesson with us, please click on the link to use our regular Lesson Enrollment form.

Please note that payment of at least half your total session fees must be made prior to the first lesson. Also note that if the fee presents a barrier for you, your family may be eligible for tuition assistance from The Village Music School Foundation: https://thevmsf.org/tuition-assistance/. If you have been a previous recipient of their assistance, please indicate that in the message here.
